Forensic human factors and ergonomics
HFN provides expert opinions on the human factors causes of transportation (road, marine, rail) crashes. Issues include visibility, visual search, perception of closing speed, information processing, expectation, perception-reaction time, light adaptation, fatigue, distraction, alcohol and drug effects.
HFN provides root cause analysis investigations of medical errors related to user-system issues in all healthcare environments. Investigations may relate to any aspect of the system including errors arising during medication preparation, medication administration, the use of medical technology and electronic health records, scheduling and transporting patients, lab and specimen collection and testing and infection control.
HFN provides expert opinions on job suitability for injured workers in occupational work settings. This may involve supporting/disputing job physical demands as reported by the employer/WSIB. Critiques and/or second opinions of existing reports (e.g., engineering, ergonomics) can also be provided.
