Crash Pattern Analysis and CountermeasuresA systems approach to crashes includes consideration of operators, vehicles, environment, and organizational influences. The majority of crashes have operator error as a contributing factor. Operator error arises from human perceptual and cognitive limitations. An understanding of the nature of operator error, as well as how it arises during interaction with the vehicle and the environment, can be used to select appropriate countermeasures.
